Case history
Mr. Barker was a forty-four-year-old company director. He worked twelve hours a day and often worked at weekends as well. He had not had a vacation for three years. He argued frequently with his wife and children. He went to his general practitioner complaining of tiredness and headaches. The doctor examined him and found mildly raised blood pressure but no other abnormality. The doctor suspected that Mr. Barker was under stress but he was very busy so he did not encourage his patient to talk. He gave him some pills for the blood pressure. Unfortunately the pills made Mr. Barker feel sick so he stopped taking them. His general practitioner suggested that he try alternative medicine.
Mr. Barker saw a therapist who talked with him for an hour. She explained that the headaches, tiredness and blood pressure were all due to stress, and that this was affecting his family relationships. She arranged for him to have some biofeedback to reduce the blood pressure. She saw him for an hour every week for counseling. She persuaded him to reduce the hours he worked, to eat a healthier diet, and to start doing yoga for relaxation and fitness. Mr. Barker’s blood pressure quickly returned to normal; his headaches disappeared; and his relationships with his wife and children improved.

1. В начале Вашей работы с материалом данного раздела рекомендуем Вам подробно ознакомиться со структурой раздела. Посмотрите, в какой последовательности и в каких рубриках расположен лексический материал. Изучите также порядок расположения речевых моделей, Это поможет Вам более успешно и целенаправленно осуществлять поиск необходимых языковых и речевых единиц при выполнении коммуникативных заданий.
2. Очевидно, Вы обратите внимание на то, что последовательность рубрик в блоке речевых моделей отражает алгоритм действий врача при общении с пациентом. Этот алгоритм также весьма четко просматривается и в сюжетах диалогов и текстов данного раздела, которые мы рекомендуем Вам прочесть прежде, чем Вы приступите к выполнению коммуникативных заданий.
3. Коммуникативные задания можно разделить на группы. Во-первых, часть из представленных в них ситуаций происходит в поликлинике или на дому у пациента, а другая часть - в стационарном отделении больницы. Во-вторых, симптомы пациентов относятся к патологическим состояниям трех систем - дыхательной, пищеварительной, сердечно-сосудистой. Рекомендуем Вам выполнять коммуникативные задания с учетом этого деления на тематические блоки, т.к. это облегчит поиск необходимых лексических единиц и речевых моделей (см. п. 1).
4. Настоятельно рекомендуем Вам использовать также известные модели речевого этикета, способы передачи эмоционального состояния и реплики, выражающие ответную реакцию слушателя, которые сделают Ваши диалоги более естественными и содержательными. В этой связи обращаем Ваше особое внимание на блоки речевых моделей "Patient's Emotions while Describing his Condition", "Doctor's Showing Interest, Asking for Additional Information, Consolation, Reassuring the Patient", "Doctor's Recommendations", "Patient's Possible Replies".
5. Надеемся, что в ходе выполнения коммуникативных заданий Вы научитесь грамотно вести опрос пациента на английском языке, взаимодействовать с ним в ходе физикального обследования, заполнения истории болезни, планирования Ваших совместных действий в получении данных для постановки диагноза, а затем в определении стратегии и тактики лечения. Постарайтесь создать психологически комфортную для пациента обстановку, в то же время будьте настойчивы в случае, если необходимо убедить пациента принять Вашу точку зрения с целью сохранения его здоровья.
6. Если Вы успешно справились с выполнением большинства коммуникативных заданий, предлагаем Вам самостоятельно или под руководством преподавателя разработать комплексные сюжеты ролевого моделирования, например: "Прохождение обследования у разных специалистов в поликлинике", "Поступление больного в приемное отделение, заполнение истории болезни, консилиум врачей отделения по поводу его диагноза и лечения", "Обход палат отделения врачами” и т.д. Кроме того, рекомендуем Вам работу в видеозале по "озвучиванию" на английском языке видеосюжетов "В поликлинике" и др.
7. Вы также можете использовать и иной способ работы с материалами данного раздела, если он представляется Вам более эффективным.
