Nevertheless, complications do occur following second-stage urethroplasty with fistula formation, glans dehiscence, and meatal stenosis (Fig. 31.13a–c), all being reported and indeed 30% of patients reported complications following second-stage urethroplasties.70

In a follow-up of their experience, Andrich and Mundy reported particularly in BXO patients that there is a tendency to recurrence in the marsupialized segment of the urethra and consideration needs to be given to a perineal urethrostomy as a more reliable management of

full length urethral strictures, particularly in the elderly patient.71 This certainly has been our experience over the last 20 years and Palminteri and Barbagli have also reported this.72

Conclusion

The first operation is likely to be the most successful, and as surgeons working in this area we should always consider the simplest

technique, which is likely to be most effective, avoiding substitution wherever possible. As a rule, for bulbar urethral strictures, an initial dilatation or urethrotomy is worth considering, but if this fails then urethroplasty should be offered and if a patient does not wish to follow this path then they should be offered urethrotomy or dilatation followed by intermittent self-dilatation for 3–6 months prior to re-eval- uation of the situation. It is unlikely that intermittent self-dilatation in the long term will

cure recalcitrant strictures, but will maintain a patent lumen.

If substitution is being considered,then a penile onlay flap for strictures in the urethra is appropriate. Grafts are now more commonly used than flaps for substitution urethroplasty,particularly in the bulbar urethra.Scrotal skin should not be used because of the high morbidity associated with it, now that there are viable and appropriate alternatives. There is a higher morbidity with the use of flaps than grafts, and both have similar efficacy.
