Figure 8.2  A 43-year-old male with carpal tunnel syndrome and right paratracheal and hilar adenopathy. The carpal tunnel syndrome was the result of caseating granulomas found at surgery. In the non-HIV-positive population, adenopathy is more common in children and non-Caucasian adults, particularly in African-Americans and persons from India. Although asymmetric adenopathy is uncommon in sarcoidosis, the patterns of tuberculous adenopathy may mimic exactly those of sarcoid and lymphoma.

The patient was anergic at admission and the diagnosis was made initially by a positive smear at bronchoscopy. He then remembered that a friend had been sick with tuberculosis several months earlier (Figure 8.11 shows the contact’s films).

Figure 8.5  (a–d) A 25-year-old male with cough and fever. There is right paratracheal, right hilar, and aortopulmonary adenopathy as well as a left pleural effusion (b). A chest radiograph 4 months earlier (a) shows that these findings are new. All of these findings favor primary infection. The patient was lost to follow-up until he returned 2 months later with continued fever and a 30 lb. weight loss. The adenopathy and pleural effusion resolved, but there are nodules disseminated throughout both lungs (c, d); the sputum was positive for acid-fast bacilli (AFB). No apparent cavitary focus is present and the nodules, now larger than 3 mm, are too big to be considered miliary. Presumably, this represents progression of the hematogenously disseminated primary disease, or primary progressive tuberculosis.

Figure 8.8  A 32-year-old male presented with chest pain, fever, and a 20 lb weight loss the day his sister was discharged from the hospital for treatment of active pulmonary tuberculosis. Multiple areas of cavitation are noted in the left lung, which is partially collapsed in the presence of a pneumothorax. The pneumothorax probably resulted from rupture of a necrotic focus into the pleural space. Smear and culture were positive for M. tuberculosis.

Figure 8.9  (a, b) Residual pleural focus. A 45-year-old female with a history of fever and pleurisy 7 years earlier treated in China for 3 months with streptomycin, isoniazid, and penicillin. A lateral chest film and computed tomography show a pleural-based soft-tissue mass without evidence of bony destruction. No parenchymal lesions or adenopathy were evident. A needle biopsy was negative; caseating granulomas were found at surgery, and M. tuberculosis was cultured. The differential diagnosis includes pleural tumors, both benign and malignant.
